Older adults and carers – understanding people’s needs as they age.
NHS Nottingham and Nottinghamshire are looking at how best to try to prevent or slow deterioration of health and "frailty". Much of this is not medical, but may be about other factors, like supporting independence or avoidance of social isolation. We are also working on better identification of health issues and reducing repeated appointments and admissions to hospital. Additionally, we want to understand how best to help people multiple health conditions. The final aspect is end of life and supporting choice and control in planning for end of life and support with compassion and dignity according to a persons' wishes.

The survey will close on Tuesday 11th March 2025 at 23:59.

NHS Nottingham and Nottinghamshire are in the process of reviewing the Care Homes Team (NHS) services, with the aim of creating a single model to replace the four different offers currently in practice countywide. As a resident, carer or family member of a resident who may have received these services, they are really keen to hear your thoughts about your experiences.
This survey is regarding the service received from the care home service team only. Across Nottingham and Nottinghamshire, these services have different names.
A summary of the different names are below:
- Mid Nottinghamshire Team
- Bassetlaw: Bassetlaw Care Homes Team
- Nottingham City: Integrated Care Homes Team, Specialist Nurses

Technology Enabled Care
As part of NCC’s Adults Technology Enabled Care (TEC) Strategy 2024 – 2027, NCC are aiming to help people use their own digital devices (e.g. smartphones, tablets, smart speakers/screens) for social care needs, like making voice-activated calls or using mental health apps. This lets people use familiar devices instead of new systems from the Council.
Feedback is welcomed from anyone over 18 with social care needs or caring responsibilities who owns a smart device. Please complete this survey to share what devices you have and how you use them.

We are pleased to announce that as part of the Integrated Mental Health Strategic Plan (Integrated Mental Health Pathway Strategic Plan 2024/25-2026/27) we will be holding our third partnership workshop event. Following discussions from our previous system-wide workshop, we are bringing partners together to explore what a local Mental Health & Wellbeing Hub could look like, with an initial focus of Mid Nottinghamshire (Mansfield, Ashfield, Newark & Sherwood). This session will focus on how we can build on existing local assets, care, and support services across health, local authorities, and the voluntary, community, and social enterprise (VCSE) sector to create a more integrated and accessible approach to mental health and wellbeing.
Workshop Focus:
✅ What should a Mental Health & Wellbeing Hub look like in Mid Notts?
✅ How can we maximise the strengths of existing services and community assets?
✅ What role should different sectors play in shaping and delivering support?
✅ How can we ensure accessibility and sustainability for those who need it most?
This is an opportunity for professionals, community leaders, and those with lived experience to co-develop a shared vision and practical next steps for a hub that truly meets the needs of the local population.
Places are limited and we are asking that people reserve their space by filling in this form.

Notts Healthcare Trust are looking for carers’ involvement reviewing assessments of the support provided to carers by their teams. This involvement could include peer review meetings, or feedback in a variety of ways. The amount of time you can offer is completely up to you.
